Treatment of dental cyst

A dental cyst is a pathological cavity near the root of a tooth, which is filled with fluid, pus or ichor. Most often it is formed due to the inflammatory process caused by periodontitis. If left untreated, the cyst destroys bone tissue, causes pain and other unpleasant symptoms. Dentists typically use two approaches to treat cysts: medical and surgical. Surgery is performed in more complex cases when the cyst reaches a large size or causes significant bone destruction.

Causes of dental cysts

One of the common causes of cyst development is deep caries. Gradually, it has a negative effect on the dentin structure, leading to an infectious process and inflammation of the pulp. This threatens the formation of a granuloma, which is subsequently filled with a mixture of pus and ichor.

In addition, a dental cyst occurs due to the following reasons:

  • Pulpitis
  • Parodontitis and periodontitis
  • Poor quality dental treatment and dental canal treatment
  • Frequent ENT diseases (sinusitis, tonsillitis)
  • Insufficient oral hygiene
  • Infectious infection of root canals
  • Incorrectly installed prosthesis
  • Jaw fracture, tooth injury

To prevent complications, it is important to consult a doctor, undergo an examination and follow all recommendations.

Causes of dental cysts

Classification

Doctors classify dental cysts according to two main characteristics: location and causes of formation.

Depending on location

The main types of cysts by location include:

  • Wisdom tooth cysts that occur in the area of the third molars
  • Formations caused by improper installation of dental crowns. They lead to improper distribution of the chewing load and the development of inflammatory processes
  • Cysts that form under the influence of infectious diseases, such as pharyngitis
  • Formations that are localized in the area of the front teeth. They are often associated with injuries or caries

In rare cases, patients are diagnosed with a calcifying cyst. With this diagnosis, the formation is filled with turbid liquid and is clearly visible on x-rays. A cyst can form anywhere in the jaw, but most often it is found in the lower part. Treatment requires resection.

Depending on the causes

Depending on the causes of cysts, there are:

  • Residual. Occur after poor-quality removal of one or more teeth, where residual tissue becomes a source of infection
  • Eruption cysts. Develops in children during the eruption of milk or molar teeth
  • Retromolar or paradental formations. Caused by the growth of wisdom teeth
  • Radicular cysts. Considered a complication of periodontitis
  • Primordial or keratocysts. Occur due to degeneration of the tooth-forming tissue

Each type of cyst has its own characteristics and requires an individual approach to treatment. For example, radicular cysts often require surgery. Formations caused by inflammatory processes in the gums are eliminated during treatment of the underlying disease.

Cyst symptoms

The departure of the gum from the tooth and exposure of its root manifests itself clinically in a variety of ways. Each of these symptoms may indicate certain diseases in the oral cavity.

One of the common signs of recession is bleeding and swelling of the gums, as well as changes in their color. This may be an indicator of periodontitis.

Another manifestation is the peeling of the gum from the tooth, the formation of pockets. This may indicate an advanced stage of periodontitis, often accompanied by purulent formations and bad breath, which increases the risk of tooth loss.

Pale or whitish gums may be a symptom of periodontitis, which is characterized by impaired blood microcirculation and leads to thinning and weakening of the gums.

Increased sensitivity, especially to temperature or sweets, is a consequence of exposed roots. This increases the risk of developing caries in these areas.

If teeth become loose, this may indicate serious damage to periodontal tissues.

Localized gum recession affecting one or two teeth often occurs due to mechanical trauma, such as improper use of toothpicks.

Why is a dental cyst dangerous?

Although a cyst over a tooth seems harmless, it can lead to serious complications. Among them:

  • Suppuration and inflammation of cystic tissues, causing an infectious process
  • Spread of inflammation to the lymph nodes, which worsens overall health
  • Damage or destruction of bone tissue in the area of the diseased tooth, which affects the stability of the dentition and jaw as a whole

The chronic process can spread to the maxillary sinuses and cause sinusitis. It also threatens the development of lymphadenitis, osteomyelitis, and periostitis.

If the protective capsule of the cyst bursts, this creates favorable conditions for the spread of pathogenic microorganisms to neighboring tissues. The source of inflammation increases, which worsens the patient’s well-being.

In addition, you must be aware that if left untreated, the affected area of bone may become more fragile. This increases the risk of spontaneous jaw fracture.

General information about dental cysts

Diagnostics

First, the doctor conducts a detailed survey of the patient and a thorough examination of the oral cavity, which helps to identify possible causes and symptoms of diseases. If a dental cyst is suspected, he issues a referral for radiography. This method allows not only to determine the size of the formation, but also its exact location.

To obtain a more detailed result about the condition of the dental system, an orthopantomogram and 3D tomography are performed. On X-rays, a granuloma usually appears as a dark spot with unclear edges. The cystic capsule has clearer boundaries and is usually located at the apex of the tooth root.

Before surgery, patients are prescribed a standard set of laboratory blood tests. These include checking blood clotting, determining blood type and Rh factor, measuring glucose levels and identifying possible infections. These measures are necessary to guarantee the safety and effectiveness of surgery.

Methods of treatment of dental cysts

Dentists use a variety of methods to treat dental cysts. A conservative approach involves performing the following steps:

  • Oral cavity sanitation. Cleaning of purulent contents, fluid and damaged tissue
  • Endodontic treatment. Involves the use of an antiseptic and the introduction of specialized drugs. They destroy the cyst membranes and stimulate bone tissue regeneration
  • Canal filling and restoration. After eliminating the cyst, the doctor closes the canals and restores the crown of the tooth
  • Antibiotic therapy and symptomatic treatment. To eliminate pain and inflammation, the patient is prescribed antibiotics and painkillers
  • Follow-up control. Six months after therapy, a control x-ray is recommended to assess the results of treatment

If conservative methods are ineffective, surgical techniques come to the rescue. The most common methods include:

  • Cystectomy. Removal of the cyst along with part of the tooth root. This procedure is performed in case of extensive damage to the root system of the tooth
  • Cystotomy. Partial removal of the cyst, especially if it is large
  • Hemisection. Removal of part of the tooth affected by the cyst, including the crown part

In cases of extensive surgical treatment of a dental cyst, the use of osteoplastic materials to restore the jaw bone is required.

The key point after treatment of a dental cyst is regular monitoring of the condition of the tooth to avoid relapse.

Will the tooth be saved?

Treatment of a dental cyst without removing it is considered possible and advisable in the early stages. Important aspects are the size of the cyst and the extent of damage to surrounding tissue. For example, a dental canal cyst or a molar cyst can be effectively eliminated with endodontic treatment. After the infection is removed, the canals are filled with a special material, such as gutta-percha. This rubber-like substance with good insulating properties tightly fills the canals, preventing re-infection. Additionally, a cementing material is used to seal the canal. It protects the tooth from bacteria and strengthens its structure.

When a wisdom tooth cyst is detected, surgical intervention is most often performed due to the difficulty of access and limited space for treatment. Surgery is considered under conditions such as:

  • Complete destruction of the tooth crown
  • Insufficient root length to restore tooth function
  • Destruction of the bone cell 2 mm or more below the gum edge
  • Impossibility of endodontic treatment due to the characteristics of the root canals (narrow, curved)

The operation to remove a cyst on the gum of a tooth is considered a complex procedure that requires a highly qualified specialist and a well-equipped clinic. After the intervention, the doctor observes the patient’s condition for some time, then gives recommendations on oral care.

Recovery after surgery

In the process of recovery after oral surgery, a number of recommendations must be followed:

  1. Avoid thermal procedures (visiting a bath or sauna, hot baths). They can increase swelling and inflammation
  2. Limit chewing on the operated side to prevent additional irritation and pain
  3. Take anti-inflammatory medications and antibiotics regularly as prescribed by your doctor to reduce the risk of infection and reduce inflammation
  4. Apply cold compresses to the cheek to reduce swelling and pain
  5. Reduce alcohol consumption and stop smoking to speed up the healing process
  6. Use antiseptic mouth rinses to prevent infections

In the first three days after surgery, it is important to avoid brushing teeth in the surgical area. As swelling decreases and pain disappears, you can gradually return to normal oral hygiene.

Possible complications after surgery include:

  • Bleeding, especially in people with bleeding disorders
  • Infections in the operated area, the treatment of which involves antibacterial therapy
  • Root fracture or crack requiring tooth extraction
  • Cracks or fractures of the jaw bone, which are rare but serious

To minimize the risk of complications, experienced dentists strictly follow all medical protocols and standards.

I would like to express my deep gratitude to Alexey Andreevich and his assistant, unfortunately I forgot to clarify her name) They are simply masters of their craft, I have never had such pleasant visits to the dentist, absolute professionals in their field, they removed two wisdom teeth at once, I did not even notice how the process went. The doctor explained everything, answered all questions and gave recommendations, and the whole process took place during a pleasant conversation. I recommend everyone to contact only him!

As a patient, I can see the doctor's attention and attitude during the appointment and pain relief. What I saw was that Yukova M.R. is a responsible, attentive and caring doctor who helped me save a tooth from extraction. Today, during an appointment, another dentist-orthopedist in another clinic took a picture and praised the work and asked for contacts where and who treated this tooth. Top class! Thank you again!!!

Thank you very much to Dr. Omar for the parental happiness that the doctor gave us!... Where and to whom we took Maxim... We had hysterics, both our Maxim and we were under enormous stress. In other clinics, doctors sang songs to him, danced and showed him all kinds of cartoons, everything was useless. They only told us to do general anesthesia..... And now we get to Dr. Omar.... MAGIC!!!.... Maksimka leaves the doctor with her teeth cured and makes an appointment to meet again as soon as possible!.... So we cured all our teeth with a smile and great gratitude. Every week he asks to meet with the doctor, just to see if new holes have appeared there..... DOCTOR FROM GOD!!!.... I wish there were more such doctors, not only for children, but also for adults. With great respect and love, your regular clients clinics (Zaitsev sisters, Tatiana and Elena)

I went to Sergei Olegovich to remove my eights. The case was difficult: they did not erupt, grew strictly horizontally, sat deep in the gums, and even very close to the nerves. I was terrified of deleting them. First, I came to Sergei Olegovich for a consultation. He looked at my picture, immediately described all the nuances of the extraction, and advised me to remove the teeth under sedation, which I am very happy about. At the second appointment, I was put to sleep and I woke up without teeth and without memories of the operation. Sergei Olegovich described all the “side effects” of rehabilitation by day, when what will hurt and how, when there will be swelling and what to do in these situations. Everything happened exactly according to the timing that he predicted. Within a week, all the swelling went down and everything went away. I definitely recommend this doctor.
