Treatment for edentia

This is the name of a dental defect, which is expressed in the partial or complete absence of teeth, caused by gum disease or injury. Congenital adentia also occurs. As it develops over time, the gums sag, bone structures are destroyed, the remaining teeth become loose and the distance between them increases.

When chewing dental units are destroyed, the digestion process is disrupted, diction deteriorates, and the effect of sunken cheeks appears, which greatly ages the face.

The most dangerous complication of edentia is the consequences. The bone tissue of the jaw atrophies, and the process of prosthetics becomes more complicated. In this case, the price of full dental implantation increases due to preliminary treatment.

To do this, the doctor performs soft tissue plastic surgery, sinus lifting, argumentation, i.e. builds up bone tissue to the required volume for subsequent implantation and prosthetics.

If necessary, before surgery, the doctor treats caries, pulpitis or periodontitis, and performs sanitation of the oral cavity. The remains of decayed teeth are also removed and periodontal pockets are cleaned.

Possible types of prostheses

Prosthetics with implants is a modern way to restore teeth and the best choice if all teeth are missing.

Full dental implantation is more profitable than installing single implants, and also looks more aesthetically pleasing. The structure consists of a base on which the prosthesis is attached. At the attachment points, the metal implant is connected to the crowns on the abutment.

In the non-removable version, the connection point is rubbed with dental cement; in the conditionally removable version, it is placed on a screw structure.

If the patient is missing all teeth, the doctor determines:

  • Removable denture Made of adhesive or implant-based plastic that must be removed for periodic cleaning.
  • Fixed Permanent prosthesis made of plastic or metal ceramics on implants.

What is the difference between the upper and lower jaw

When installing a prosthesis on the entire jaw, the specialist takes into account structural differences:

  1. The bone below is stronger and denser. The healing process takes up to 4 months. If there is not enough volume in terms of thickness or height, bone grafting is performed
  2. The bone is looser on top. Nerves and maxillary sinuses are located close to it. In case of atrophy, the bone must first be built up. More base implants are required. Healing occurs over 5–6 months

If the teeth have been missing for a short time and bone resorption has not occurred, treatment and prosthetics occur faster.

Basic implantation technologies

Classic technology

Prosthetics occurs in stages. First, the specialist prepares the patient’s oral cavity, treats dental diseases if necessary, and removes remaining roots. Then the implants are installed.

A few months after they have healed, the doctor puts in a temporary prosthesis, and after habituation and adaptation, a permanent one.

It is permissible to install up to 14 implants per jaw. During the procedure, the gum is cut and metal pins are screwed into the alveolar process of the bone crest. Installation occurs in certain places at right angles under local anesthesia.

An important condition is the height of the bone tissue - from 5 mm. With plastic surgery and extensions, implantation (for good engraftment) is carried out after 6 months.

Prosthetics with a permanent prosthesis made of metal or metal-ceramics is possible 3–6 months after implantation. This is important so as not to immediately load the bone. During this time, the doctor sews up the incision sites on the gum. A week before the installation of the adaptive prosthesis, special formers are placed.

The classic method is the longest; it takes 6 to 9 months before installing a permanent prosthesis. In this case, it is possible to obtain a good aesthetic effect and an even gingival contour.

One-stage implantation

The technique is also called basal; the implants are immediately loaded with a prosthesis. In this case, metal roots are installed deeper into the cortical layers of the jaw.

The design itself is simpler than with the classical method. The implantologist selects the number of pins - from 6 to 10 per row, the location of fixation, and the installation feature (at a right angle or at an angle). The gum is not cut, but pierced. Bone grafting is not performed; 2.5 mm of bone is sufficient.

With one-stage implantation, after installing titanium rods, a temporary plastic prosthesis can be installed on days 2–3. The patient wears it for about 1–2 years, after which the structure is replaced with a permanent one made of zirconium alloy or metal ceramics.

After implantation, the patient can evaluate the immediate effect. If the gum contour is not smooth, the defect is eliminated using artificial gum.

All-on-4 technology

One-stage protocol, but only 4 supports are placed on one jaw: two short ones vertically and two long ones (at an angle). The manipulation is carried out without an incision, and with a puncture of the gum, the implant is implanted into the deep layers of bone tissue.

The technique is effective for atrophy and bone thickness from 2.5 mm. If the deficit is greater, an increase is carried out.

The all-on-4 design includes 10 elements on four supports. 3 days after implantation, a temporary plastic prosthesis is placed with immediate loading. After 1 year it is also replaced with a permanent one.

All-on-6 technology

A variation of the previous one, but with the installation of six support pins. Performed in case of bone tissue resection for better stabilization of the prosthesis.

3 days after implantation, the doctor installs an adaptive prosthesis with 14 elements. After one year, it is changed to a permanent one with metal-ceramic or zirconium crowns.

Cost of full dental implantation

In case of complete edentia, before prosthetics, the patient must undergo an examination and consultation with a dentist-implantologist. The doctor issues a referral for tests, a panoramic image of the jaw, and a computed tomography scan. After examination and evaluation of diagnostic results, the specialist makes a decision on the method of treatment and offers the patient possible options.
