About the service

If it is impossible to deliver drugs through the Eustachian tube, transtympanic administration is used by injecting the eardrum. The method is widely used in the treatment of middle ear diseases and allows the drug to be quickly transplanted directly into the pathological focus.

Transtympanic injection of drugs (MD) has considerable advantages. Thanks to the direct introduction of medicinal solutions into the tympanic cavity, the expected therapeutic effect is quickly achieved:

  • Relief of inflammation
  • Cessation of exudate secretion
  • Restoration of hearing function

Pronounced effectiveness is also ensured by the absence of losses due to transplantation of the administered drug compared to oral and other types of administration. This allows the use of small doses of drugs during conservative therapy.

The method is characterized by low trauma using local anesthesia. The risk of complications is minimal.

Indications

Transtimapanal injection of drugs is an effective way to combat symptoms indicating middle ear pathology. These include:

  • Acute serous otitis
  • Suppurative otitis media
  • Meniere's disease
  • Sensorineural hearing loss in the acute stage

Indications for eliminating clinical symptoms are:

  • Loss of hearing acuity (unilateral, bilateral)
  • Ear congestion
  • Ear pain of varying intensity
  • Periods of severe dizziness
  • Noise in the ears when changing body position
  • Isolation of exudate from the auditory canal

Taking into account the clinical manifestations, the doctor selects the necessary medicine for transtympanic delivery: for purulent-inflammatory forms of otitis - antibiotics, in the case of sensorineural hearing loss - corticosteroids.

The manipulation is also performed to supply a contrast solution into the tympanic cavity for the purpose of subsequent radiographic diagnostics.

General information about the procedure

Preparation

There are no specific preparations for the procedure. The manipulation is performed on an outpatient basis by an otolaryngologist. In this case, the patient is positioned lying on his side, providing access to the sore ear.

How is the procedure?

The doctor treats the affected ear with an antiseptic solution, after which the soft tissues are numbed. In this case, solutions of phenol, lidocaine or anesthetic gel applications are used.

Once the anesthesia has taken effect, the doctor uses a syringe with a thin needle to pierce the eardrum or use a perforated hole resulting from the pathological process. The contents of the tympanic cavity are carefully aspirated.

Then a medicinal substance with a volume of 0.8-1 ml, heated to body temperature, is injected into the freed space. The needle is removed, the perforated area is treated with an antiseptic, and a swab soaked in an antibacterial solution is placed in the ear canal.

After completion of the procedure, the patient is recommended to remain in a lying position for another 15-20 minutes to avoid dizziness and spatial disorientation. Depending on the severity of the disease, one-time or course treatment is used.

Rehabilitation

After the therapy, it is recommended to stop using headphones for 2-3 weeks, avoid hypothermia, overheating, and swimming in public bodies of water. The patient is advised to take vitamin complexes and restorative drugs, and physiotherapeutic procedures.
