Health resort card form 076/y is a document that is required for children who go on vacation or treatment to health resorts and some health camps.

This is a medical document, where the pediatrician notes the absence of restrictions for spa treatment. In addition to medical reports and recommendations, the card contains: information about the patient, documentary data of the child (number of the compulsory medical policy (MHI) and information about vaccinations); information about disability, benefits (if any). Conclusions and recommendations are filled out on the basis of medical documents and examinations passed.

Required examinations

To get a sanatorium card, a child needs to pass a number of tests and be examined by narrow specialists appointed by a pediatrician:

  • Pediatrician consultation.
  • Blood and urine analysis.
  • Analysis for enterobiasis.
  • Examination by a dermatologist.
  • ECG.
  • Chest X-ray.
  • Consultation of a specialized doctor for a disease requiring special treatment.

A feature of filling out a modern sanatorium-resort card is the new rules of Rospotrebnadzor in connection with the spread of the new coronavirus infection COVID-19 - the presence of an analysis for coronavirus.

Thanks to the data in the card, the attending physicians in the sanatorium prescribe procedures and a course of treatment aimed at restoring and maintaining the health of the child.

When making a spa card, doctors use the international designation (code) of diseases. Therefore, this card is suitable for treatment not only in Russia, but also in any country.

The spa card is valid for 1 month. It is recommended to start issuing a card 2-3 weeks before a trip to a health institution.

After the completion of treatment, a reverse tear-off coupon is filled in the sanatorium card, which is provided to the doctor who issued the sanatorium card.
