What is endoscopic sinusotomy?

During the operation, an endoscope, a thin and flexible instrument, is inserted through the nasal passages. It allows the doctor to monitor the condition of the sinuses and remove pathological formations that contribute to the development of unpleasant symptoms.

What is endoscopic sinusotomy?

Contraindications

The main contraindication is active respiratory infections: if they are present, surgery should be postponed until complete recovery. Other contraindications:

  • Severe blood disease. Hemophilia and other conditions associated with poor blood clotting increase the risk of bleeding during and after surgery
  • Severe forms of allergies (for example, Quincke's edema). Against this background, the sensitivity of the nasal mucosa increases

Surgery is not performed against the background of serious problems with the heart and lungs: this is due to the fact that the presence of these problems increases the risk of complications during surgery.

Indications for surgery

Endoscopic sinusotomy is prescribed against the background of chronic sinusitis. In this condition, inflammation of the paranasal sinuses continues for more than 12 weeks. This disease is accompanied by constant congestion and pain in the face.

Another indication is fungal infections of the paranasal sinuses.

Other indications:

  • Chronic rhinitis Long-term inflammation of the nasal mucosa, in which a runny nose does not go away even after washing and using sprays. Symptoms: nasal congestion, heavy discharge and frequent sneezing
  • Nasal polyps Benign soft growths on the nasal mucosa or sinuses. They cause blockage of the nasal passages, complicate breathing and reduce the sense of smell
  • Deformation of the nasal septum An anatomical deviation in which the nasal septum is so curved that it makes it difficult to breathe normally through the nose and contributes to the development of sinusitis
General information about the procedure

Preparation

Before the operation, the otolaryngologist performs an endoscopic examination of the nasal structures. To clarify the diagnosis, a CT scan of the paranasal sinuses may also be required.

Additionally, the patient needs:

  • Do general and biochemical blood tests
  • Have a coagulogram
  • Take a chest x-ray
  • Make an electrocardiogram (ECG)
  • Consult an anesthesiologist

If the patient is taking medications that affect blood clotting, he should discuss with his doctor the possibility of temporarily stopping them.

How the endoscopic sinusotomy operation is performed

The operation of endoscopic sinusotomy usually takes 1-3 hours. It is performed in a hospital setting or on an outpatient basis. The main stages look like this:

  1. Preparation. The patient is given local or general anesthesia
  2. Insertion of the endoscope. It is administered through the nasal passages
  3. Surgical intervention. Using special instruments inserted through an endoscope, the doctor removes polyps, corrects deviated septums, or removes other obstructions that interfere with normal drainage and ventilation of the sinuses

After eliminating the problem areas, the endoscope is removed. Taking into account the fact that all manipulations are performed through natural openings, stitches are usually not applied. If there are no complications, the patient goes home a few hours after the procedure.

Result of surgery

Most patients experience a significant reduction or complete disappearance of headaches, nasal congestion and facial pressure. Normal breathing through the nose is restored, the sense of smell improves, and the overall quality of life increases.

After surgery, the risk of developing recurrent sinusitis and other inflammatory sinus diseases is reduced. In most cases, the results of surgical intervention last for many years.

Rehabilitation after surgery

The speed and success of recovery from surgery varies depending on the complexity of the procedure. In order for the body to recover faster, it is necessary to avoid physical exertion, overheating, consumption of too hot or spicy foods and alcohol.

You also need to take medications as prescribed (the doctor prescribes antibiotics and painkillers). Normalization of nasal breathing usually occurs within 10 days.
