Feature of the procedure

The ear, throat and nose are interconnected by a common system of internal passages. This feature is used in otolaryngology to restore the function of air exchange between these organs in the process of treating inflammatory diseases. With an acute respiratory viral infection or a runny nose, an infection can enter the middle ear and cause otitis media, which is accompanied by pain, congestion and hearing loss. The procedure of blowing the ears helps to get rid of fluid, restore the patency of the auditory tube, and prevent the development of purulent otitis media and other complications.

The manipulation is carried out using an ENT combine with a pneumatic gun attachment or mechanically. During the procedure, air flows from the nose through the nasopharynx and eustachian tube to the eardrum. Ear blowing is also a diagnostic method. If there is an obstacle in the path of air flow in the form of an impassable plug of mucus or liquid, this helps the doctor make an accurate diagnosis and develop subsequent treatment tactics.

Feature of the procedure
Indications for the procedure

ENT recommends that the patient blow through the ears through the nose in case of suspected obstruction of the auditory tube due to frequent recurrent otitis.

The procedure is prescribed for:

  • Tinnitus
  • Headaches
  • Ear congestion
  • Feeling of water in the ear canal

Manipulation is carried out after suffering tubo-otitis, eustachitis, purulent otitis, in order to assess the patency and air exchange between the tympal cavity and the middle ear. Politzer blowing is often prescribed for exudative otitis media. The procedure is also performed after tympanoplasty or before surgery on the hearing organ.

Contraindications

Blowing pipes according to Politzer is not carried out when:

  • Damage to the eardrum
  • Barotrauma after deep sea diving
  • Mental illnesses, epilepsy
  • Exacerbation of inflammatory diseases of the ear, nose and throat (infectious rhinitis, laryngitis, pharyngitis)
  • Pathologies of the inner ear

Infectious diseases of the nose and nasopharynx in the acute stage are a contraindication due to the risk of infection in the middle ear and aggravating the situation. You can blow out your ears only after the inflammation has been relieved with medication during the period of stabilization of the patient's condition.

Preparation for the procedure

There is no need to specially prepare for blowing the ear through the nose. Immediately before the procedure, the nasal passages should be cleared of mucus and crusts. During the appointment, the doctor instills vasoconstrictor drops or a spray into the nose to eliminate swelling. Blowing the auditory tubes according to Politzer to a child is painless, but you should explain the procedure to the child in advance and reassure him.

Helpful information

How to blow out ears

After preparing the nasal passages, the patient sits in a chair straight, holding his head straight. An otoscope is inserted into the ear, and the tip of the Politzer device is pressed tightly into the nostril on the same side. It connects to a rubber bulb to perform manual pressure. If an ENT combine is used, then a pneumatic gun nozzle is installed in the nasal passage, into which air is pumped from the device through a tube.

The doctor does the blowing in three approaches. The patient pronounces a word of 3 syllables, most often “pa-ro-hod,” stretching the vowels. At this time, air is forced through the nozzle into the ear and exits through the ear canal. Another common name for this ear procedure is steamboating. The patient listens carefully to sensations and sounds that may indicate the following:

  • Intense hum and crackling noise - normal patency of the auditory tube
  • Weak noise and clicks - there is an obstacle to the passage of air in the auditory tube (cerumen plug, mucus)
  • Gurgling, the sound of bursting bubbles - there is fluid or purulent exudate in the middle ear

The feeling of a buzzing in the ears and dizziness is a normal reaction to blowing through the nose; it goes away on its own, so it does not require special correction. If, during the ENT procedure, a steamer appears sharp pain and deterioration of the condition, you should tell the doctor about this, he will stop the manipulation.

Blowing the ear according to Politzer takes 5 minutes (on one side). In total, you should complete a course of 5–10 sessions. Usually the doctor prescribes procedures daily or every other day.

The effect of the Politzer nasal blowing procedure

Improvement is noted after the first session, but the result can be fully assessed after completing a course of 10 manipulations.

The “steamboat” procedure at an ENT specialist for otitis media has the following effects:

  • Normalization of air exchange between the tympal cavity and the middle ear
  • Improving fluid outflow
  • Relieving ear congestion
  • Reducing inflammation and pain

After a course of blowing out the eardrum in combination with drug treatment, hearing is partially or completely restored. If the procedure does not bring results, the doctor may order catheterization of the auditory tube.

Complications that can be caused by blowing out the ears “steamboat”

If the procedure was performed poorly or without taking into account contraindications, the following may occur:

  • Spread of infection into the middle and inner ear. Occurs if the manipulation was carried out during an exacerbation of the infectious process in the nasopharynx or during acute otitis
  • Emphysema. Air from the nose enters the subcutaneous tissue of the nasopharynx. Occurs extremely rarely due to violation of procedure technique
  • Nosebleed. Damage to the vessels of the mucous membrane occurs when very active manual blowing of the nose
  • Barotrauma with damage to the eardrum. Possible with very intense air flow and increased pressure

In our clinic, “steamboat” ear blowing using modern ENT equipment is carried out by qualified doctors with extensive experience. Before the procedure, the patient undergoes a comprehensive diagnosis using otoscope, rhinoscope, tests. Manipulation is prescribed if necessary for control treatment, in order to reduce the possible risk of complications and adverse reactions to therapy. If any side effects occur, the doctor will immediately provide emergency assistance.
