The management of locally recurrent prostate cancer after radical radiotherapy remains controversial due to the perceived high incidence of severe genitourinary (GU) and gastrointestinal (GI) toxicity associated with any method of salvage radiotherapy.
Luca F Valle et al conducted a systematic review and meta-analysis of the data. The review did not find large differences in 5-year outcomes when comparing all salvage treatments (salvage therapy). Reirradiation with SBRT, HDR brachytherapy, or LDR brachytherapy appears to result in less severe genitourinary toxicity than radical prostatectomy, and reirradiation with HDR brachytherapy results in less severe GI toxicity than radical prostatectomy.
